OneMedical Group is seeking a Care Navigator to help patients access the right care and services. You will receive, review and process patient requests, ensuring information is recorded accurately and directed to the appropriate clinical or administrative pathway.
You will help ensure patients experience an efficient, supportive and seamless journey when using our services, working with clinical and administrative colleagues to manage requests and improve processes.

As a Care Navigator, you will help ensure patients receive an efficient, supportive and seamless experience when accessing our services.
You will support patients by signposting them to appropriate practice, community, external or self-referral services, while recognising when concerns need to be escalated to a clinician.
Working closely with both clinical and administrative colleagues, you will help ensure patient requests are managed safely, efficiently and within appropriate timescales.
We're looking for someone with excellent communication and organisational skills who can remain accurate and professional while working in a busy environment.
