Job summary

Home Instead Crawley is seeking a compassionate Care Coordinator to oversee care services across Crawley, Horley and East Grinstead. You will coordinate schedules, liaise with clients, carers, and GP practices, and ensure care plans meet regulatory standards.

Strong IT skills, excellent communication, and a caring, organised approach are essential. A Level 3 NVQ in Health & Social Care is preferred and a valid driving licence is advantageous for travel within the territory.

Qualifications

• Excellent communication skills and the ability to build rapport quickly.
• Proficiency with IT systems, Microsoft Office and virtual platforms.
• Experience with care management apps such as Access Care Planning or Birdie preferred.
• NVQ Level 3 in Health and Social Care or equivalent is preferred.

Responsibilities

• Coordinate care schedules across Crawley, Horley and East Grinstead.
• Liaise between clients, carers and GP practices to address needs.
• Prepare and maintain rotas considering travel time and holidays.
• Ensure care plans and records comply with regulations.
• Audit care notes and support continuous improvement.
