Agincare in the New Milton area is seeking a Care Coordinator to organise and match carers with clients, coordinate care services and plan rotas to ensure continuity of care.
You will also support the setup of new care packages, communicate with families and healthcare professionals, and participate in the on-call rota with additional pay. The role offers a salary of £26,000–£28,000 and various benefits.
