Overview

Royal Primary Care Chesterfield – We have an opportunity for 1 x part-time (22.5 hours per week) Care Coordinator based at Royal Primary Care East sites, based in Chesterfield.

You will join a growing establishment committed to being a flagship organisation and an at scale provider of primary care services in the Midlands. If you are enthusiastic, caring and passionate about delivering the highest quality care, keeping patients at the heart of all you do, we want to hear from you.

An opportunity has arisen to join our growing team at Royal Primary Care and we are looking for an enthusiastic part-time Care Coordinator to work within our busy medical practice within the Chesterfield area focusing on supporting patients who are in respite care at a local Care Home.

Responsibilities
  • Manage the registrations and prescription management of a busy respite care home.
Qualifications and Skills
  • Excellent IT skills and good prioritisation abilities.
  • Understanding of General Practice processes or relevant transferable skills.
  • Ability to work as part of a team.
  • Good level of qualification (or equivalent experience).
  • Competency in Maths and English.
