We are seeking an organised, compassionate and proactive Care Coordinator to join our domiciliary care team. You will play a vital role in ensuring high-quality care is delivered to our service users by effectively scheduling care visits, supporting care staff, and maintaining strong communication with clients and families.
Coordinate and schedule care visits using care management systems
Match carers to service users based on skills and preferences
Handle rota planning, including sickness and emergency cover
Communicate with clients, families and care staff professionally
Ensure care plans are accurate and up to date
Support onboarding and training of care staff
Monitor compliance with CQC standards and company policies
Handle enquiries and concerns in a timely, empathetic manner
Maintain accurate records and reports
Take part in On Call duties
Previous experience in domiciliary care coordination (preferred)
Strong organisational and time management skills
Excellent communication and people skills
Confident using scheduling and care management software
Calm under pressure and able to manage competing priorities
Knowledge of CQC regulations and care standards
A compassionate and professional approach
Must be a driver with a vehicle
