Job summary

Home Instead is seeking a Care Coordinator who will real-time coordinate and optimise care plans, staying closely aligned with families, care staff and healthcare providers. You will monitor outcomes, maintain detailed client records, and communicate scheduling changes promptly. The role requires GCSEs, scheduling software proficiency, and a valid driving licence.

• GCSEs including Maths and English are required.
• Proficiency with scheduling software and Microsoft Office.
• Valid driving licence and access to own vehicle may be required.

Responsibilities

• Coordinate and optimise care plans in real-time with families, care staff, and healthcare providers.
• Proactively monitor and adjust care plans to improve client outcomes.
• Maintain meticulous client records and care documentation, ensuring accuracy and compliance.
• Collaborate with the Care Manager and team to address client needs and concerns.
• Respond to schedule changes and communicate with all parties to keep care delivery running smoothly.
• Manage administrative tasks including data entry and report generation.
• Provide professional telephone support to clients and families.
• Contribute to the On Call rota to ensure continuity of care during critical moments.
• Champion compliance with healthcare regulations and company policies.
