Main duties of the job

We are looking for a highly motivated Care Coordinator to lead the procurement of nursing home placements and/or domiciliary care for patients clinically assessed for Hospice Rapid Access Service.

The post will develop relationships both in-house and with external organisations, to ensure that the patient has access to the most appropriate care environment for their holistic needs. The candidate should manage timescales sensitively and have a good understanding of GDPR and confidentiality.

You will be a team player, motivated, forward thinking and a talented individual interested in delivering excellent specialist palliative care within the Hospice Rapid Access Service team

Job responsibilities
  • To co-ordinate the procurement of Nursing Home and Domiciliary Care for Mid Essex patients at the end of their lives (last12 weeks)
  • To be responsible for data collection for all aspects of placements utilising SystmOne records and computerised databases
  • To work closely with all clinical teams at Farleigh Hospice and other provider organisations to deliver the above service andto escalate clinical concerns and sourcing difficulties to the Rapid Access to Discharge Manager
  • To support the Rapid Assessment and Discharge Team with administrative support as requiredThrough telephone, virtual and face-to-face discussions, provide information to patients with palliative and end of life careneeds, supporting their families and carers, liaising with clinical professionals and referring on as necessary to appropriatehealth and social care partners
  • To support the Hospice in building strong relationships with external partners and stakeholders and promote the goodreputation of the Hospice e.g. local trusts, ICB, health and social care partners, businesses and voluntary organisations
  • To reconcile the invoices against care delivered to ensure accurate balances and raise with care providers if there anyreconciliation discrepancies
Person Specification
Qualifications
  • A levels or equivalent
  • Administrative, leadership or management qualification or equivalent training
Experience
  • Experience in an administrative, navigational or procurement role
  • Experience of managing people and supporting services
  • Experience of maintaining and keeping accurate records and date entry
  • Knowledge of palliative services
  • Experience of procuring and negotiating services
  • Experience of multi-disciplinary or multi-agency team working
  • Experience of budget management
  • Experience of working within a clinical service
  • Experience of managing datasets and presenting information when necessary
Skills and Knowledge
  • Proven ability of successfully working with people and handling difficult and sometimes complex situations
  • Ability to work under pressure, manage own stress and emotional responses and be aware of stress in others
  • Ability to use a range of IT systems and software, including Word, Excel, PowerPoint, Office 365 and electronic patient record systems
