A public healthcare provider in Fordingbridge seeks an enthusiastic Clinical Pharmacist with IP qualification to join their friendly Pharmacy team. An exciting opportunity has arisen for an enthusiastic and forward-thinking Clinical Pharmacist with IP qualification to join our friendly Pharmacy team who are embedded in four GP surgeries within Avon Valley PCN. We are a supportive, proactive and forward-thinking Pharmacy team with an excellent clinical lead. We serve a population of around 40,000 patients across four sites, The Fordingbridge Surgery in Fordingbridge, The Ringwood Medical Centre and Cornerways Surgeries in Ringwood and Twin Oaks Surgery in Bransgore. The practices have been working together for some time and have a good relationship with each other and with the well-respected pharmacy team.
The clinical pharmacist will work with the existing pharmacy team to support the safer delivery of medicines and support medicines optimisation across the PCN. We are keen for the clinical pharmacist to have the opportunity to use their prescribing qualification. They will:

AvonValley Primary Care Network is an umbrella organisation delivering care to40,000 patients, across four GMS Practices based in the New Forest area. ThePractices include The Fordingbridge Surgery in Fordingbridge, The RingwoodMedical Centre and Cornerways Surgeries in Ringwood and Twin Oaks Surgery inBransgore. The practices have been working together for some time and have agood relationship. Together we aim to build robust services across our networkto improve patient care and are keen to bring in new members of the primarycare team to achieve these goals.
We are fortunate in havingboth Pharmacists and Technicians already established and embedded in thepractices who are employed both by the West Hampshire Clinical CommissioningGroup as well as the Primary Care Network. Both teams work closely together toimprove medicines optimisation for all patients across the PCN. The pharmacyteam works using an innovative model whereby the day-to-day clinical pharmacytasks for all four practices are pooled and dealt with as one. They alsoprovide support for the GPs, and carry out medicine optimisation work,medication reviews as well as some long-term condition clinics. We are keen toexpand on this expertise to lend more support to long-term condition managementincluding setting up CVD prevention clinics shortly across our network.
Tobe part of a multi-skilled primary care team in a patient facing role providingpatient centred medical services based on patient needs with a focus oncontinuity of care.
