Job summary

Oxford Health NHS Foundation Trust is seeking a capable District Nurse to join our community teams across Oxfordshire on a fixed-term basis for one year to cover maternity leave. You will work in people's homes, delivering high-quality, evidence-based nursing care and coordinating with carers and multidisciplinary teams.

You will have autonomous practice with access to ongoing professional development and leadership support within a district nursing framework.

Qualifications

• Registered General Nurse (adult) with current NMC registration.
• Educated to degree level or equivalent.
• Full UK driving licence and access to own vehicle.

Responsibilities

• Holistically assess, plan, deliver and evaluate personalised care.
• Manage allocated caseload and coordinate referrals.
• Deliver a broad range of community nursing interventions.
• Promote independence and wellbeing through patient education.
• Collaborate with healthcare professionals to prevent hospital admissions.
• Supervise and develop staff, students and carers.
• Maintain clinical governance and documentation compliance.
