Lincolnshire Community Health Service NHS Trust is seeking a Community Nurse to join the Louth and Wolds District Nursing Team on a full-time permanent basis. The role involves delivering nursing care to patients in their homes, care homes and clinics, with autonomous practice and teamwork within the MDT.

You will assess care needs, plan, deliver and evaluate care using evidence-based practice, and participate in ongoing professional development.

Competenze

• Current 1st level UK NMC adult nurse registration
• Evidence of continuous professional development
• Mentorship, supervisor or assessor qualification and its use in practice
• Post registration qualifications e.g. palliative care, wound care, diabetes
• Evidence of continuous professional development
• Evidence of completed leadership modules/qualification
• Evidence of V100 or V150 Community Practitioner prescribing course undertaken
• Post remigration qualifications in elderly care, continence or related areas

Mansioni

• Assess care needs, plan, deliver and evaluate nursing care
• Travel to patients' homes and clinic settings
• Work autonomously and as part of a district nursing team
• Coordinate care with MDT (Social Workers, GP and Allied Health Professionals)
• Participate in induction, preceptorship and clinical skills programmes
