Oxford Health NHS Trust in Oxfordshire is seeking a dedicated Community Nurse to join our district nursing locality. You will assess, plan, deliver and evaluate holistic care in patients’ homes, working autonomously within a supportive team to prevent hospital admissions.

We support your progression through a district nursing career framework, including advanced clinical practice and leadership training, with regular 1:1 support, development, and annual appraisal.

Formación

• Essential: Registered General Nurse (adult).
• Essential: Educated to degree level or equivalent.
• Desirable: Driving licence and access to own vehicle.
• Desirable: Prescribing qualification.

Responsabilidades

• Assess, plan, deliver and evaluate holistic, person-centred care for patients with planned and urgent healthcare needs.
• Manage and prioritise an allocated caseload with timely intervention and discharge planning.
• Deliver a wide range of community nursing interventions and respond to changing patient needs.
• Collaborate with patients, carers, multidisciplinary teams and partner agencies to maximise independence and reduce hospital admissions.
• Provide leadership, supervision and support to staff, students and carers, contributing to workforce development.
• Maintain high standards of clinical governance, record keeping, safeguarding and compliance with policies.
• Contribute to service improvement, audits, and the efficient use of resources.
