Whether you are an experienced nurse looking for a new challenge or newly qualified community nursing is for you!
We would be delighted to offer the opportunity to join our innovative, dedicated, and supportive team delivering outstanding care within the South-West Oxfordshire district nursing locality.
Our friendly teams take pride in supporting their team to deliver proactive, caring, safe and excellent standards of patient care to their local communities.
Our community nurses work autonomously, delivering high quality and evidence-based nursing care, see problem-solving as a positive challenge, and are creative in many varied and unpredictable clinical situations.
The District Nursing service in Oxfordshire is at the heart of developments in community care, as per the NHS Long Term Plan. A crucial part of the integrated community plans is recognition of the value of the District Nurse Specialist Practitioner Qualification for District Nursing Team Leaders, as advocated by The Queen’s Nursing Institute, The King’s Fund, and the RCN which we can support you to work towards.
The service currently operates 7 days a week between the core hours of 0800-1830. Shift patterns available are varied and can be discussed at interview with a flexible approach.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
- Assess, plan, deliver and evaluate holistic, person-centred care programmes for patients with planned and urgent healthcare needs.
- Manage and prioritise an allocated caseload, ensuring timely intervention, risk assessment, referral and discharge planning.
- Deliver a wide range of community nursing interventions, recognising deterioration and responding appropriately to changing patient needs.
- Work collaboratively with patients, carers, multidisciplinary teams and partner agencies to maximise independence, reduce hospital admissions and support safe discharge.
- Provide leadership, supervision and support to staff, students and carers, contributing to workforce development and a positive learning environment.
- Maintain high standards of clinical governance, record keeping, safeguarding and compliance with organisational policies, procedures and professional standards.
- Contribute to service improvement, audit, quality initiatives and the effective use of resources to deliver safe, efficient and responsive community care.
We will support your progression through a District Nursing career progression framework, including advanced clinical practice and leadership training in this area.
