Job summary

Oxford Health NHS Foundation Trust is seeking a District Nursing Lead (Advanced District Nurse Specialist) to drive clinical leadership, implement anticipatory care models, and collaborate with GPs and multi-disciplinary teams in Oxfordshire. The post sits within Planned and Preventative Care, with driving responsibilities, and requires driving licence/vehicle access.

Qualifications

• Advanced historical taking or comprehensive assessment of the frail person.
• DNSP qualification (District Nursing Specialist Practitioner).
• Non-medical prescriber (V300) or willingness to undertake module.
• Desirable: Community Practice Teacher preparation.
• Desirable: Coaching and facilitating learning module.

Responsibilities

• Provide expert clinical leadership to District Nursing teams.
• Lead implementation of the national anticipatory care model with GPs and multi‑professional teams.
• Conduct advanced clinical assessment, prescribing, and care pathway development.
• Support staff to deliver safe, effective care and manage competing priorities.
• Promote professional standards and a culture of learning and quality.
