Oxford Health NHS Foundation Trust is seeking an experienced or newly qualified community nurse for a fixed-term, one-year maternity leave cover within the South West district nursing locality. Our teams deliver high-quality care in people’s homes in a supportive, innovative environment.

You will holistically assess, plan, deliver and evaluate personalised care, manage a caseload, and work with multidisciplinary teams to prevent hospital admissions and support timely discharge.

Qualifikationen

• Holistically assess, plan, deliver and evaluate personalised programmes of care.
• Manage an allocated caseload, prioritising planned and urgent needs.
• Deliver a broad range of community nursing interventions and education.
• Promote independence and wellbeing through patient education and rehabilitation.
• Work with healthcare professionals to prevent admissions and support discharge.
