Job summary

Qualifications

• Full valid driving licence and access to a vehicle for work.
• Experience in community nursing is desirable.
• Evidence-based practice and high standards of patient care.

Responsibilities

• Assess, plan, deliver and evaluate holistic, person-centred care programmes for patients with planned and urgent healthcare needs.
• Manage and prioritise an allocated caseload with timely intervention, risk assessment, referral and discharge planning.
• Deliver a wide range of community nursing interventions, recognising deterioration and changing patient needs.
• Work with patients, carers, multidisciplinary teams and partner agencies to maximise independence and safe discharge.
• Provide leadership, supervision and support to staff, students and carers, contributing to workforce development.
