Responsibilities

• Assess, plan, deliver and evaluate holistic, person-centred care for patients.
• Manage and prioritise an allocated caseload with timely interventions.
• Deliver community nursing interventions and respond to changing patient needs.
• Collaborate with patients, carers and multidisciplinary teams to support safe discharge.
• Provide leadership, supervision and support to staff and learners.
• Maintain high standards of clinical governance, safeguarding and record keeping.
• Contribute to service improvement, audits and efficient use of resources.
