The Royal Berkshire NHS Foundation Trust is seeking a Band 3 Assistant Patient Pathway Coordinator to support the Rheumatology team. This part-time post (30 hours weekly) is on-site and focuses on coordinating bookings and managing the patient pathway with a proactive, customer-focused approach.

You will work within a multidisciplinary team to maintain appointments, schedules, and patient information, ensuring timely care from referral to discharge and adherence to SOPs and clinical targets.

Qualifications

  • Experience using IT systems and patient data systems.
  • Experience scheduling outpatient appointments.
  • Experience working with patients and conveying information about appointments or treatments.
  • Experience working in a healthcare setting.

Responsibilities

  • Be the point of contact for all administrative issues relating to patients' pathway of care.
  • Support the patient pathway from referral to discharge within a multidisciplinary team.
  • Contribute to high levels of patient and clinician satisfaction as a customer-focused contact point.
  • Follow Standard Operating Procedures and defined timescales for role activities.

Band 3 Assistant Patient Pathway Coordinator (CAT 9 - Rheumatology)


This is a part-time post for 30 hours per week.


An exciting opportunity has arisen for a motivated individual to join our Rheumatology Team to assist the Patient Pathway Manager and Patient Pathway Coordinators by carrying out delegated responsibilities for a range of operational activities, working autonomously using initiative and their specialist knowledge and experience of the service to make decision.


PLEASE NOTE: This is an on-site role and is not suitable for remote working.


Main duties of the job

To specialise in specific modality appointment bookings but have an overview and knowledge of all booking processes.


To ensure full compliance with scheduling guidelines, protocols and clinical targets whilst identifying and resolving breach situations and the consequences of changes to timetables and potential impact on capacity.

Job responsibilities

To be the point of contact for all administrative issues relating to patients pathway of care.


To work as part of a multidisciplinary team and provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.


To ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact.


To provide a consistent approach across the Trust; Standard Operating Procedures (SOPs) will describe how the functions of the role will be carried out, along with the timescales to which are to be adhered.

Person Specification

Qualifications


  • GCSE or equivalent

  • Educated to A-level, or NVQ Level 4 in Business Administration/Customer Care or acquired equivalent experience


Experience


  • Experience of using of IT systems and patient data systems

  • Experience of scheduling outpatient appointments and/or admissions

  • Experience of working with patients and providing information regarding their appointments or treatment team

  • Experience of working in healthcare setting
