The Workington Community Rehabilitation Team are looking to appoint an Assistant Practitioner or a candidate who is willing to work towards the Assistant Practitioner qualification. The role involves working within a delegated caseload, conducting day‑to‑day assessments, liaising closely with the ICC MDT and other agencies on treatment progression, patient safety, and ongoing care.

Main responsibilities
  • Provide support and assistance to registered clinicians in the delivery of community rehabilitation and nursing services.
  • Support in assessment and management of patients, respecting privacy, dignity, individuality and promoting independence and wellbeing.
  • Own a delegated caseload of patients, prioritise workload based on team pressures and patient needs.
  • Use specialist skills and knowledge to provide flexible care for patients with physical, psychological, social and environmental needs to maintain independence.
  • Implement interventions that enable patients to achieve personal goals, using continuous assessment and adjustment of clinical interventions. Evaluate effectiveness of interventions.
