A community rehabilitation organization based in Sherborne is looking for an Assessment and Support Coordinator to join their Community Rehabilitation Team. The Community Rehabilitation Team (CRT) West supports people living with complex mental health problems to achieve their goals, along with gaining the skills and confidence to live as independently as possible.
CRT West works as an MDT, discussing, formulating and delivering individualised plans. We are looking for a dynamic and enthusiastic Assessment Support Coordinator to join the team, full time. You will need to work creatively to deliver a high standard of care that optimises a person's own health, wellbeing and recovery. We work in a responsive way to provide stability in a person's life and help them to develop meaningful occupation alongside managing the symptoms of their mental illness.
The role of social worker in the team will lead on all aspects of social care, and will require experience, or keen interest, in working within a specialist service. Knowledge and application of the Care Act, Mental Capacity Act and Mental Health Act is essential to the role.
CRT West covers the footprint of the Dorset Council Locality, and therefore access to a vehicle is essential.
