Advanced Nurse Practitioner/Advanced Clinical Practitioner

We are looking for an Advanced Nurse Practitioner to join our Nurse‑led, innovative, friendly, supportive General Practice in Hanwell W7.

This is an exciting opportunity for the right person to join our team to deliver exceptional services to our patients. To work with multidisciplinary team with support of Nurse Practitioner Director, Nursing team, GPs and administrative team. They can also lead on modernising patient pathways, supporting autonomous clinical decision‑making, and enhancing care delivery across a progressive, community‑focused practice.

Main duties of the job
  • NMC registration
  • MSc in Advanced Practice (or equivalent)
  • Primary care and minor illness experience
  • Familiarity with systmOne medical system wouldbe an advantage
  • Confident in managing full patient episodesautonomously
  • Strong leadership, communication, anddecision‑making skills

Job responsibilities
  • To provide Nurse Practitioner services to patients of the practice, including diagnosis, treatment, screening and advice within the professional competence of the post holder.
  • To provide clinical leadership including acting on Clinical Governance issues, such as urgent and routine results, letters and on instructions from outside agencies such as hospitals and consultants.
  • To work independently in the surgery in the absence of a GP, possessing the initiative and confidence to do so.
  • To work as part of the Primary Healthcare Team, sharing decision‑making and information where appropriate.
