Job description

We are looking for a motivated and experienced Advanced Nurse Practitioner to join our friendly and supportive team. This is an exciting opportunity to work autonomously while being part of a multidisciplinary team committed to delivering high-quality, patient-centred care.

The post holder will provide advanced clinical assessment, diagnosis, treatment, and management of patients, contributing to service development and quality improvement. You will use your advanced clinical skills to support patients across the service, working closely with GPs, consultants, nurses, and allied health professionals.

Main duties of the job

The Advanced Nurse Practitioner will work autonomously within the GP practice to assess, diagnose, treat, and manage patients presenting with acute, chronic, and complex conditions. The post holder will provide same‑day and routine appointments, undertake comprehensive clinical assessments, request and interpret investigations, and prescribe independently within their scope of practice.

The role includes management of long‑term conditions, health promotion, and preventative care, as well as signposting and referral to appropriate services. The ANP will work closely with GPs and the wider multidisciplinary team to deliver safe, effective, and patient‑centred care, contributing to service development, clinical audit, and quality improvement activities.

The post holder will practice in accordance with NMC standards, local policies, and evidence-based guidelines.

Job Purpose: The Advanced Nurse Practitioner will work autonomously and collaboratively within the GP practice to provide high-quality, patient-centred care. The role involves assessing, diagnosing, treating, and managing patients with acute, chronic, and complex conditions, using advanced clinical skills and independent prescribing.

The ANP will contribute to service development, quality improvement, and staff education, supporting the wider multidisciplinary team to deliver safe, effective, and integrated care.

Key Responsibilities / Duties

Clinical Responsibilities:

  • Conduct comprehensive clinical assessments of patients presenting with acute, chronic, or complex conditions.
  • Make diagnoses, formulate management plans, and prescribe medications independently within scope of practice.
  • Manage long-term conditions, including monitoring, review, and patient education.
  • Provide same‑day and routine appointments for patients, ensuring timely, patient-centred care.
  • Interpret investigations and make appropriate referrals to secondary care or other community services.
  • Promote health and wellbeing, providing advice and support on prevention, lifestyle, and self‑care.

Leadership and Service Development:

  • Support service development, quality improvement, and clinical audit projects.
  • Contribute to policy and guideline development within the practice.
  • Mentor and supervise junior staff, student nurses, and other trainees as appropriate.

Professional Standards:

  • Maintain professional registration with the NMC and comply with relevant legislation, guidelines, and local policies.
  • Ensure clinical practice is evidence-based and aligned with national standards and best practice.
  • Participate in continuous professional development and reflective practice.

Qualifications
  • Essential Qualifications
  • Registered Nurse (NMC) with current registration.
  • Postgraduate qualification in Advanced Clinical Practice or equivalent (Masters level preferred).
  • Independent prescribing qualification (V300 or equivalent).
  • Evidence of advanced clinical assessment, diagnostic, and decision-making skills.
  • Additional training in leadership, service development, or teaching/mentorship is desirable.
Experience
  • Proven experience in primary care, urgent care, or a relevant clinical setting.
  • Experience managing acute and chronic conditions autonomously.
  • Experience working within a multidisciplinary team.
  • Experience in clinical audit, quality improvement, or service development is desirable.
