Job Overview

The Trust offers a new and exciting opportunity to join the Waltham Forest Rapid Response Team. Rapid Response Team is a progressive and innovative multidisciplinary team aimed at supporting the population of Waltham Forest, providing urgent MDT response to prevent avoidable hospital admissions. The team works across 7 days a week, 24 hours a day.

The aim is to support and treat suitable patients in the community, with a range of escalated health and social care needs.

Responsibilities
  • Work collaboratively with health and social care professionals across the borough and Integrated Care System.
  • Lead clinical decision making and undertake enhanced holistic assessment of patient and family needs in partnership with the patient, family and carer.
  • Triage referrals from other agencies or disciplines.
  • Co‑ordinate intensive and complex nursing care with other multi‑disciplinary agencies.
  • Appoint to have management experience.
  • Acute care experience required.
  • Be proficient in advanced clinical skills and knowledge with the ability to assess physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient, their carers and the wider MDT as needed.
  • Work collaboratively and carry out joint visits members with MDT colleagues within the team to promote holistic person‑centred assessment and approach to care.
  • Participate and complete managerial and clinical supervision.
  • Provide line management supervision to members of the Rapid Response multidisciplinary team.
  • Contribute to audits within the service.
  • Conduct and contribute to Datix investigations / Root Cause Analysis / Serious Incidents and help support lessons learnt within the service.
  • Support and mentor pre‑registration learners from a variety of disciplines.
  • Be a driver and have access to a car.
Qualifications

Essential Criteria

  • Registered Nurse (Part 1 of NMC register) or Health & Care Professions Council e.g. Paramedic.
  • Non‑Medical Prescriber.
  • Postgraduate level study or demonstrated post‑graduate level working evidence via continuous professional development.
  • Significant previous post‑registration experience in the relevant key areas at Band 6 or above.
  • Experience of working in a community or urgent care response team.
  • Experience of managing in a customer‑focused environment, providing professional support within a busy environment.
  • Demonstrate skills to organise, prioritise clinical caseloads within a multidisciplinary team.
  • Demonstrate up‑to‑date knowledge and expertise in the speciality 0‑2hr urgent care response.
  • Demonstrate a clear vision of the role and the service.
  • Demonstrate proficient information, written and oral, in a clear and logical manner.
  • Reliability, adaptability with excellent interpersonal and communication skill.

Desirable Criteria

  • Certificate / Diploma in Management Studies Specialist Community Practice Teacher or equivalent.
  • Experience of quality improvement processes and clinical audits.
  • Knowledge and understanding of the health needs of a culturally and ethnically diverse urban population with levels of deprivation.
  • Proficient IT skills (Word, Data entry or IT qualification or equivalent).
  • Demonstrate compassionate and followership leadership skills.
Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement – London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
