Job summary

Harrow Health CIC is seeking a Patient Coordinator to manage ADHD patient pathways from referral to discharge, coordinating referrals, registrations and triage in line with Right to Choose requirements. The role supports timely assessments, treatments and follow-up while safeguarding patient safety.

You will work with multidisciplinary teams to ensure smooth communication between patients, GPs and clinicians, provide welfare calls, and signpost to support services, while maintaining high

Qualifications

• GCSE English and Maths (or equivalent)
• Good written and verbal communication
• Willingness to complete statutory and mandatory training
• Basic IT literacy, ability to use electronic systems and Microsoft Office
• Customer service or an equivalent qualification
• Qualification or training in Social Prescribing
• Additional training or qualifications in healthcare administration or patient coordination

Responsibilities

• Coordinate referrals and manage patient pathways from referral to discharge.
• Validate clinical and administrative documentation and arrange appointments.
• Monitor waiting times and pathway progression against KPIs.
• Coordinate shared care communications with primary care and clinicians.
• Provide welfare calls and non-clinical support to patients and carers.
• Signpost to local ADHD support services and wellbeing resources.
• Maintain confidentiality, information governance and safeguarding standards.
• Use EPR systems to manage patient information and appointments.
