Job summary

The Patient Coordinator role sits within the Right to Choose Adult ADHD Assessment and Treatment Service. You will manage patient pathways from referral through assessment, treatment, monitoring and discharge, ensuring an accessible, patient-centred service across the NHS.

You will coordinate referrals, arrange appointments, maintain records, support communication between patients, clinicians and GPs, and provide welfare calls and signposting to relevant services while safeguarding

Responsibilities

• Receive, review and process referrals through the electronic referral system.
• Coordinate appointments and waiting lists; ensure pathway progression.
• Maintain accurate patient records and information governance practices.
• Communicate with patients, clinicians and GPs throughout the care pathway.
• Provide welfare calls and signposting to ADHD support services.
