NHS Dumfries and Galloway is seeking a proactive member of the Patient Flow Team based at Dumfries & Galloway Royal Infirmary and Regional Community Hospitals. You will coordinate patient journeys and support timely, safe discharges from hospital to home or other care settings.
Working across acute and community services with a multidisciplinary team, you will remove barriers to discharge and ensure patients receive the right care, in the right place, at the right time.

Working across acute and community services as part of a multidisciplinary team, you will play a pivotal role in coordinating patient journeys and supporting timely, safe discharges from hospital to home or other care settings across Dumfries and Galloway.
This is a rewarding and challenging role where no two days are the same. You will work closely with a wide range of health and social care professionals to overcome barriers to discharge, coordinate complex care arrangements, and help ensure patients receive the right care, in the right place, at the right time.
The successful candidate will have excellent communication, organisational and problem-solving skills, with the ability to build effective relationships across services and work confidently in a dynamic environment.
