Qualifications

• Bachelor's Degree in Nursing.
• 3–5 years of experience in Case Management or related healthcare setting, preferably international.
• Fluent in English; additional languages (French, Portuguese, or German) are assets.

Responsibilities

• Member advocacy and care coordination with members, providers and services.
• Assess needs and develop personalised care plans.
• Monitor progress and ensure safe transitions of care.
• Educate and empower members for self-management.
• Collaborate with internal and external teams to deliver programmes.
• Maintain accurate case records and ensure quality of care.
• Promote value-based, cost-effective healthcare solutions.
