Job summary

Qualifications

• Bachelor’s degree in nursing and active professional registration.
• 3–5 years of experience in case management or related healthcare setting, preferably international.
• Fluent English; additional languages (French, Portuguese or German) are a strong asset.
• Proficient with healthcare management systems and MS Office.

Responsibilities

• Advocate for members and coordinate care across providers, hospitals and services.
• Assess medical, psychosocial, and functional needs and develop care plans.
• Monitor progress, support safe transitions of care, and collaborate with clinicians.
• Educate members on conditions, options, resources and self-management.
• Deliver programmes and ensure quality, documentation and compliance.
