Zusammenfassung

Jobtailor is seeking a dedicated care manager to develop, assess, and coordinate complex care plans for members with mental and behavioral health needs. You will conduct outreach by phone and in-home visits, build ongoing care plans, and connect members with providers and community resources.

The role requires a Master’s degree in Behavioral Health or Social Work or a nursing degree, with 2–4 years of related experience and appropriate licensure.

• Collect, document, and maintain member information for regulatory compliance.
• Educate members and families about procedures, options, benefits, and social needs.
• Provide leadership feedback to improve cost-effective care and quality.

• Develop, assess, and facilitate complex care management activities for members with primarily mental and behavioral health needs
• Evaluate member needs by phone or in-home visits and recommend or facilitate care/service plans
• Conduct telephonic, digital, home, and other site-visit outreach
• Develop ongoing care plans for members with high acuity
• Identify providers, specialists, and community resources for mental health and substance use disorder care
• Coordinate among members, families/caregivers, community resources, and care provider teams
• Monitor care plans, member status, treatment side effects, complications, and clinical symptoms
• Facilitate care coordination and collaborate with providers and specialists to ensure timely access to care
• Collect, document, and maintain member information and care management activities for regulatory compliance
• Educate members and families about procedures, provider instructions, treatment options, referrals, healthcare benefits, and social determinant needs
• Provide leadership feedback on opportunities to improve cost-effective care and quality delivery
• Comply with all policies and standards and perform other duties as assigned

Requirements

• Master's degree in Behavioral Health or Social Work OR a degree from an accredited school of nursing
• 2–4 years of related experience
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements, with behavioral health experience required
• Preference for an active independent Kentucky licensure, especially LCSW or LPCC
• Experience with case management and mental health services, including serious mental illness (SMI) or substance use disorder
• Residence in Kentucky or bordering states Ohio, Indiana, West Virginia, or Tennessee preferred
• Ability to work Monday through Friday, 8:00 AM–5:00 PM Eastern
