Job summary

BruntWork is seeking a motivated bilingual Primary Care Case Manager fluent in Spanish and English to improve patient outcomes, engagement, and reduce unnecessary emergency room visits and hospital utilization.

The Case Manager will proactively review patient records, identify follow-up needs, contact patients, and coordinate timely appointments with primary care providers via in-person visits or telehealth.

Qualifications

• Bilingual in English and Spanish is required.
• Experience in a medical office, care coordination, case management, or patient outreach.
• Strong understanding of medical terminology and chronic conditions.
• Experience with EMR software.
• Excellent organizational and follow-up skills.

Responsibilities

• Review patient charts and identify high-risk patients.
• Proactively contact patients and schedule appointments.
• Coordinate follow-ups after ER visits or hospitalizations.
• Support chronic disease management and care coordination.
• Educate patients on preventive care and engagement.
• Document outreach and reporting in EMR with HIPAA compliance.
