SnappyCX, Inc. is seeking a remote Bilingual Care Coordinator to join our patient-centered team. In this role you will organize and monitor care plans, coordinate with patients, families, providers, and staff, and ensure timely service delivery while upholding HIPAA standards.

You will manage care tracks, update electronic records, verify insurance, and connect patients with community resources. Strong bilingual communication and empathy are essential to success in a fast-paced telehealth

Qualifications

• 3+ years of experience in healthcare coordination or medical administrative support.
• Proficiency with Electronic Health Records (EHR) systems and HIPAA-compliant communication tools.
• Excellent verbal and written communication with strong empathetic listening.
• Outstanding time-management skills and ability to manage multiple care tracks simultaneously.

Responsibilities

• Act as the central hub of communication between patients, providers, and care teams to ensure coordinated delivery of services.
• Assist in building, updating, and monitoring patient care plans and scheduling follow-up appointments.
• Maintain accurate electronic health records (EHR/EMR), process referrals, and verify insurance eligibility.
• Connect patients with community resources and medical equipment providers as needed.
• Conduct routine check-ins to monitor treatment adherence and educate patients on care plans.
• Ensure coordination activities comply with HIPAA regulations and SOPs.
• Identify delays or gaps in care and flag them for clinical leadership.
