SnappyCX, Inc. is seeking a highly organized Bilingual Patient Coordinator (English/Spanish) to support coordination of care for home health patients.

This fully remote role serves as the primary contact among patients, families, HHAs, clinicians, contracts, and internal teams. The candidate will document communications in HHA Exchange, manage cases, generate reports, and adapt to a fast-paced healthcare environment.

Formación

• Fluent in English and Spanish is required.
• Previous HHA Exchange experience is required.
• Computer savvy; comfortable with healthcare software and digital tools.
• Strong written and verbal communication skills.
• Excellent customer service and active listening skills.
• Strong organizational and time-management skills.
• Ability to multitask and manage multiple cases simultaneously.

Responsabilidades

• Coordinate work assignments based on Home Care Worker training and experience.
• Communicate effectively with patients, families, HHAs, clinicians, contracts, and departments.
• Document all communications and information in HHA Exchange.
• Monitor and resolve open cases to ensure timely resolution.
• Support patients and families by identifying needs and coordinating care.
• Communicate changes in service days/hours and updates to HHAs.
• Provide information to healthcare professionals and stakeholders.
• Complete daily, weekly, and monthly reports.
• Manage overtime and travel-time requests.
• Use tools to locate aides, provide directions, and check availability.
• Complete and document incident reports.
• Manage mutual and linked cases.
• Monitor and respond to emails, voicemails, and faxes.
• Follow agency guidelines and adapt to changes.
• Escalate concerns to the supervisor.
• Perform other duties as assigned.
