Real Hires is seeking a Case Client Coordinator to help ensure clients receive the care they need on time and without unnecessary gaps.

You’ll connect clients, families, caregivers, and the care team, keeping schedules organized and solving issues quickly when plans change.

This role emphasizes organization, clear communication, and careful record keeping in HHA Exchange. Spanish fluency is a plus.

Formación

• Experience in case coordination, scheduling, healthcare, or administration.
• Excellent communication and problem‑solving skills.
• Spanish fluency is a plus.

Responsabilidades

• Schedule and assign caregivers based on client needs and availability.
• Manage inbound and outbound calls with clients, families, and caregivers.
• Handle call-offs, coverage, and schedule changes.
• Manage referrals and service inquiries.
• Maintain accurate records in HHA Exchange.
• Monitor the Call Dashboard and assist with pre-billing.
• Ensure services align with each client’s Plan of Care.
• Report incidents and important changes to management.
• Participate in team meetings and scheduled on-call rotations.

What You’ll Do

What You’ll Bring

• Excellent communication and problem-solving skills.
• Comfortable handling high call volumes and changing priorities.
• Experience with healthcare or scheduling platforms.
• Proficiency in Microsoft Office.
• HHA Exchange experience and Spanish fluency are a plus.
