Qualifications

• Active and unrestricted RN license in New Jersey.
• Fluent in Spanish and English, both written and verbal.
• Minimum of 2 years of clinical nursing experience.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and patient engagement abilities.
• Experience with EMR/EHR systems.
• Ability to work independently in a remote environment.
• Strong organizational and documentation skills.

Responsibilities

• Conduct patient outreach and follow-up calls in Spanish and English.
• Provide nursing support, patient education, and care coordination services.
• Communicate with patients, providers, caregivers, and healthcare teams regarding treatment plans and needs.
• Document patient interactions accurately in EMR/EHR systems.
• Assist patients with understanding medications, treatment plans, and appointments.
• Support case management, utilization review, care management, intake, or clinical support activities as assigned.
• Identify and elevate clinical concerns appropriately.
• Maintain HIPAA compliance and client requirements.
• Collaborate with interdisciplinary healthcare teams to support patient outcomes.
• Participate in quality improvement and team initiatives.
