Job summary

ClearDesk seeks a Bilingual Intake and Referral Coordinator to help patients access home health care. You will handle referrals, verify eligibility, and coordinate schedules with nurses and field staff.

Your Spanish and English fluency will be central to the role, ensuring clear, compassionate communication with patients and families. You’ll thrive on structure in a changing environment, escalating questions when needed and maintaining organized records.

Qualifications

• Must be fluent in English and Spanish (verbal and written).
• Excellent organizational skills with ability to triage and prioritize referrals.

Responsibilities

• Serve as the first point of contact for incoming patient referrals and inquiries.
• Answer and manage incoming calls from current and prospective members.
• Triage referrals to determine appropriate documentation.
• Verify patient eligibility and ensure proper authorization.
• Coordinate with the authorization team to keep workflow running smoothly.
• Schedule intake appointments with nurses and field staff.
• Use internal systems to process new member information and documentation.
• Communicate with Spanish-speaking patients clearly and professionally.
• Escalate concerns promptly when additional support is needed.
