Your Opportunity:

The Registered Nurse - Transition Coordinator is a clinical consult-based position. Transition Coordinators assess patients’ needs in the context of the whole person, including their life situation and determinants of health. Critical thinking and accurate interpretation of complex information from various sources are essential for making appropriate clinical decisions regarding continuing care living options. The role involves comprehensive assessment, planning, and coordination of care, along with ongoing communication with healthcare providers across the continuum of care. Major responsibilities include completing electronic needs assessments, determining urgency/priority, developing and implementing service plans, coordinating service delivery across programs and settings, liaising with Continuing Care staff and community resources, and providing information and support to clients, families, and caregivers. The Transition Coordinator works within a multidisciplinary team and is responsible for referrals, documentation, participation in meetings, process and outcome evaluation, and professional development. Support to rural sites across the Central Zone may also be provided. Work tasks and charting are computer-based, involving extensive repetitive motion tasks.

Description:

As a Registered Nurse (RN), you will provide a wide range of nursing services to patients, families, communities, and populations, ensuring their safety and well-being. You will utilize nursing processes, including critical thinking, problem solving, and decision making, while teaching, counseling, and advocating for patients and families. You will support an integrated, holistic approach to patient care, health promotion, and maintenance, providing safe, quality, patient- and family-centered care aligned with AHS values.

Note: Provisional Permit Holders shall be paid $39.58/hour - $54.16/hour. Upon registration as a Registered Nurse, pay will be adjusted according to AHS/UNA agreements.

Required Qualifications:

  • Completion of an accredited nursing program
  • Active or eligible registration with the College of Registered Nurses of Alberta
  • Current BCLS-HCP certification
  • If not practiced in last five years, completion of CRNA Medication Management and NextGenU Health Assessment courses

Additional Qualifications:

  • At least two years community-based nursing experience, including geriatric care
  • Effective time management, communication, and conflict resolution skills
  • Ability to work independently and in teams, with strong assessment and critical thinking skills
  • Ability to handle stress, prioritize, and meet deadlines
  • Physical ability to perform patient care tasks

Preferred Qualifications:

  • Knowledge of Microsoft Office, RAI Tools, Meditech, Connect Care, and e-People is an asset
