Social Worker (MSW) Inpatient Units - Temporary Full-Time

Status : Temporary Full Time (up to 12 months with possible extension)

SALARY: $44.67 - $57.90/Hour

Position Summary:

Under the general direction of the Program Manager, the Social Worker is a member of the inter-professional team and provides assessment and individual or group treatment to clients. The Social Worker is accountable for maintaining the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers. The goal of the Social Worker is to assist clients and support their families/support network to achieve optimal client psychosocial health outcomes in collaboration with the team and other service providers. The Social Worker participates in client flow and timely discharge, supporting positive client outcomes, service efficiency and effectiveness.

Requirements:

Education/Specific Training Requirements:

  • Master’s Degree in Social Work from an accredited post-secondary institution
  • Current unrestricted registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Additional training and education related inpatient stroke care, Cognitive Behavioral Therapy, conflict resolution, and effective communication skills
  • NVCI, CPI preferred

Experience and Knowledge:

  • Two years’ experience providing Social Work service in an inpatient acute medicine or rehabilitation setting
  • Recent experience in a discharge planning and/or patient care coordination role
  • Demonstrated understanding and application of Advanced Care Planning, Substitute decision-makers, Power of Attorney processes
  • Demonstrated understanding of local and regional programs, including SE LHIN Home and Community Care, Community Health Centers and Family Health Teams and processes for accessing these services in a timely manner.
  • Excellent communication skills, in particular with patients and families experiencing cognitive decline, Aphasia and other communication barriers, and significant and complex changes in medical condition
  • Demonstrated understanding and alignment to current healthcare funding models, expectations and deliverables of the organization, including achievement of efficient patient flow and targeted lengths of stay
  • Demonstrated commitment to evidence-based practice specific to the rehabilitation, seniors health, palliative care, short and long-term complex medical care populations, including use of standardize clinical assessments and screening tools
  • Demonstrated knowledge and application in partnering with Primary Care and Community Care teams in ensuring effective transitions for patients leaving hospital
  • Demonstrated capacity to practice independently and as an effective interprofessional team member
  • Demonstrated strong interpersonal skills in developing professional relationships with clients, their families and interprofessional team members
  • Demonstrated positive approach toward innovation and change and ability to quickly adapt to evolving workplace standards of practice
  • Demonstrated ability to prioritize work effectively, manage a full workload and meet deadlines in a busy environment with frequent interruptions
  • Demonstrated trouble shooting skills and ability to problem solve issues that may arise in carrying out the duties of this position
  • Demonstrated understanding and commitment to health and safety policies, procedures and applicable legislation
  • Proven willingness to learn and acquire new information and skills
  • Demonstrated good physical condition to meet the physical demands of the position
  • Basic proficiency in Microsoft Office Suite of products including, Word, Excel and Outlook; ability to manage electronic recording, workload and meet submission timelines
  • Bilingual in French and English language, preferred
  • Good attendance and availability required
