Extendicare is seeking a Registered Practical Nurse to join our team in Canada. The RPN will provide patient-centered nursing care, administer medications, and ensure care plans are accurately documented and updated.

You will collaborate with the DOC/DHW and interdisciplinary teams to coordinate care and support residents and families. A strong emphasis on infection control, safety, and quality care is required.

Qualifications

• Current registration as a Registered Practical Nurse in applicable Province/State.
• Experience in a senior environment; working with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, or long-term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.
• Current Basic Cardiac Life Support and First Aid certification required.
• Computer proficiency with Microsoft Office applications.
• Current VPS (Vulnerable Position/Sector Screening) within 6 months.

Responsabilités

• Provide direct resident care and medication administration and document care in accordance with regulatory standards.
• Complete and ensure resident care assessments, care/service plans, and documentation are up to date; communicate changes.
• Implement and evaluate care/service plans and adjust as required.
• Coordinate nursing team assignments to ensure efficient and timely service delivery.
• Respond to resident condition changes and medical emergencies; provide emergency first aid.
• Educate residents and families on care plans and procedures.
