Extendicare is seeking a Registered Practical Nurse to deliver planned, patient‑oriented nursing care in our long‑term care settings across Canada. You will work with an interprofessional team to assess, plan, implement, and evaluate resident care while adhering to provincial standards and Extendicare policies.

You will participate in medication administration, care coordination, and education for residents and families, while maintaining documentation and regulatory compliance.

Formación

• Current registration as a Registered Practical Nurse in the applicable province/state.
• Experience working in a seniors’ environment; unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/complex/long‑term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management, and customer‑focused skills.
• Demonstrated sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification required.
• Proficiency with Microsoft Office applications.
• Current (within 6 months) VPS.

Responsabilidades

• Work as a member of the interprofessional team using Extendicare’s Practice Model and person-centered care.
• Complete resident clinical care assessments, care/service plans, and documentation.
• Implement and evaluate care/service plans and adjust as needed.
• Assist with scheduling resident care conferences and participate as applicable.
• Collaborate with DOC/DHW to facilitate moves, transfers, and admissions.
• Provide direct resident care and medication/treatment administration; document per policies.
• Safely process orders, store drugs/narcotics, and administer medications per requirements.
• Educate residents and families.
• Coordinate care plans with residents and teams; oversee assignments.
• Respond to changes in resident condition and medical emergencies.
