Job summary

Extendicare is seeking a Registered Practical Nurse (RPN) in Ontario to join our care team. Reporting to the Director of Care or Director of Health and Wellness, you will provide planned, patient-centered nursing care in accordance with provincial standards, assessments, care/service plans, and documentation.

In this role you will collaborate with the interprofessional team, administer medications, educate residents and families, and support move-ins and transfers.

Qualifications

• Current registration as a Registered Practical Nurse in the applicable province/state.
• Experience working in a seniors’ environment and with unregulated care providers preferred.
• Strong written and oral communication, critical thinking, care coordination, time management, and customer‑focused skills.
• Proficiency with Microsoft Office applications.
• Current Basic Cardiac Life Support and First Aid certification required.

Responsibilities

• Work as a member of the interprofessional team applying a person‑centered approach to care.
• Complete and ensure resident assessments, care/service plans, and documentation are accurate and up to date.
• Implement and evaluate care/service plans and adjust as needed.
• Assist with scheduling resident care conferences and participate as applicable.
• Provide direct resident care and medication/treatment administration, documenting care per regulations and policies.
• Teach, delegate to, and direct unregulated care providers.
• Coordinate nursing team assignments to ensure efficient service delivery.
• Respond to significant changes in resident condition and medical emergencies; provide emergency first aid as required.
