Job summary

Extendicare in Canada is seeking a Registered Practical Nurse (RPN) to deliver patient-centered nursing care as part of an interprofessional team. You will complete assessments, develop care plans, document changes, and participate in care conferences, ensuring safety and regulatory compliance.

The role involves administering medications, coordinating with physicians, and educating residents and families while adapting plans to evolving needs in a long-term care setting.

Qualifications

• Must be a registered practical nurse and eligible to practice in the province.
• Experience with seniors and unregulated care providers preferred.
• Knowledge of gerontology, rehabilitation or long-term care is an asset.

Responsibilities

• Provide planned, patient-centered nursing care in line with provincial standards.
• Complete assessments, care/service plans, and ensure accurate documentation.
• Administer medications and treatments per regulatory requirements and Extendicare policies.
• Coordinate care assignments and participate in resident conferences as needed.
• Educate residents and families and respond to changes in condition promptly.
