Job summary

Extendicare in Ontario is seeking a Registered Practical Nurse (RPN) to deliver patient-centered nursing care in a long-term care setting. Reporting to the DOC/DHW, you will assess residents and develop care plans, administer medications, and collaborate with the care team to ensure high-quality outcomes.

Ideal candidates bring gerontology experience, strong communication, and a commitment to safety and regulatory compliance in a team-oriented environment.

Qualifications

• Current registration as a Registered Practical Nurse in the applicable Province.
• Experience working in a senior care environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, or long-term care is an asset.
• Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills.
• Sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification required.
• Computer proficiency with Microsoft Office applications.
• Current VPS (within 6 months) screening.

Responsibilities

• Work as a member of the inter professional team applying person-centered care.
• Complete resident assessments, care/service plans, and documentation updated as required.
• Implement and evaluate care plans and adjust as needed.
• Assist with scheduling of care conferences and participate in conferences.
• Collaborate with DOC/DHW to manage moves and transfers.
• Provide direct resident care and administer medications, documenting per policy.
• Process orders and safely manage drugs and narcotics per regulations and policy.
• Educate residents and families about care and treatment.
• Coordinate care plans with physicians and team.
• Coordinate nursing assignments for timely service delivery.
• Respond to significant condition changes and emergencies; provide first aid.
• Teach and supervise unregulated care providers.
• Address resident/family concerns with leadership.
• Support Infection Prevention & Control and safety programs.
• Uphold Extendicare values and code of conduct.
• Complete other duties as assigned.
