Job summary

Qualifications

• Minimum one-year relevant experience preferably in a health care setting.
• Previous experience in a long-term care setting is preferred.

Responsibilities

• Assist with the move-in process of new residents by coordinating with residents and their POA.
• Participate in Continuous Quality Improvement (CQI) program.
• Evaluate effectiveness of Social Service Work services and provide input for updates.
