Job summary

Cencora is seeking a Field Case Manager to guide patients, healthcare providers, and program partners from a home office with occasional in-person visits. The role emphasizes coordination, patient-centered care, and reimbursement navigation, across Canada.

Qualifications

• RN or related degree (BSc in Pharmacy or other Science degrees)
• Knowledge of public or private reimbursement arena
• Bilingualism is an asset (Quebec requirement)
• Previous Case Manager experience is an asset

Responsibilities

• Serves as the primary contact for physicians.
• Communicates with patient, program stakeholders, and colleagues to maintain trusted relationships.
• Educates healthcare providers on program services, patient resources, and enrollment processes.
• Reviews patient charts and ensures completion of documentation including authorizations and renewals.
• Manages the special authorization process to ensure timely submissions and guidance for reimbursement.
• Provides ongoing communication to physicians regarding patient status and program changes.
• Adapts program elements to meet provider needs and supports in-clinic and remote care.
