Cencora is seeking a Nurse Case Manager to support patient and provider services in Canada. The role focuses on patient support, disease education, and coordinating access to therapies with healthcare teams.

The successful candidate will manage patient journeys, document interactions, and participate in training, meetings, and partnerships with pharmaceutical teams to optimize funding and access. A nursing license and provincial registration are required.

Qualifications

• Diploma in health care or related field is required or preferred.
• Licensed Nurse with active registration in the province of employment is required.
• Ability to work both independently and in a team to manage patient cases effectively.

Responsabilités

• Provide information on Patient Support services, diseases, and therapies.
• Assist patients and caregivers with assessments related to Patient Support services.
• Act as liaison between patients, families and health care providers for medication access.
• Document patient information in the electronic patient record.
• Follow case-management protocols including data collection and adverse event reporting.
• Coordinate with healthcare providers and clinics for treatment logistics and issue resolution.
