Job summary

Cencora is seeking a Senior Nurse Case Manager – Rare Disease. This remote role supports patients and providers across Canada, coordinating care, reimbursement, and monitoring. You will ensure data integrity, participate in program development, and train new case managers as needed.

The ideal candidate holds a RN or bachelor’s degree, with 5+ years of nursing and strong communication skills in French and English. The position offers comprehensive benefits and a remote work setup within Canada.

Qualifications

• Bachelor’s degree or RN diploma; an advanced degree is preferred.
• At least five years of nursing experience.
• Healthcare and case-management experience in a medical or hospital practice, insurance, or biopharmaceutical industry.
• Experience in third-party reimbursement, pharmaceutical sales, managed care, or related area is an asset.
• Ability to communicate medical information clearly and appropriately.

Responsibilities

• Provide medical support and education to patients and health care providers on disease, therapy and management.
• Deliver inbound and outbound telephone support, including maintenance and adherence calls.
• Coordinate activities and documentation to obtain prescription drug coverage through various funding programs.
• Collaborate with insurers, physician offices, and pharmacies to identify reimbursement solutions and resolve issues.
• Maintain accurate case information and complete ongoing audits of files and data.
