Overview

The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. The program supports patients and their families by connecting individuals with community-based healthcare providers, social support services, primary care, and acute care teams to create personalized plans that improve outcomes and ease transitions to home.

Job Summary

We are hiring a dynamic, passionate Physiotherapist who wants to be part of transformational change in home and community care by providing care in a truly client-centered approach. You will work in an environment that places the care team in the driving seat, with a self-directed team offering professional autonomy and responsibility. You will be part of a diverse care team serving a roster of clients in the community, conducting joint care planning with a focus on client-centered goals across physical, cognitive, social, emotional, and spiritual domains to create a holistic care plan. Each client is assigned a primary care clinician who leads care planning and facilitates the interdisciplinary team to reach the clients care goals. As part of the SE Transitions team, you will be accountable for the care and outcomes you provide and the progress your clients achieve.

Requirements
  • A graduate of an accredited physiotherapy program and a member in good standing with the College of Physiotherapists of Ontario
  • Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred
  • Reliable, flexible, creative, self-directed professional; able to work effectively with rehab assistants and a high-performance interdisciplinary rehabilitation team
  • Sound knowledge of community resources is an asset
  • Effective interpersonal, organizational, and communication skills
  • Positive work and attendance record
  • Ability to adapt to new computer applications
  • Hybrid work approach with remote work and client interactions in acute/rehab or community-based rehabilitation settings
  • Vehicle and valid drivers license and ability to travel
