Job summary

CBI Home Health in Peterborough, Ontario is seeking an Occupational Therapist to support patients at home through functional assessments, interventions and home modifications within our Integrated Care Program.

You will collaborate with care teams, provide ADL/IADL strategies, and use digital platforms for care delivery, documentation and coordination. A valid OT license with the College of Occupational Therapists of Ontario is required, and travel within the community is expected.

Qualifications

• Must be registered with the College of OccupationaI Therapists of Ontario.
• Ability to travel within the community with a valid driver’s license and insurance.
• Experience in home care and integrated care programs is preferred.
• Strong communication and collaboration skills.

Responsibilities

• Provide OT interventions and home modifications, including ADL/IADL strategies.
• Collaborate with interdisciplinary care teams to deliver integrated care.
• Support restorative approaches and health coaching.
• Utilize digital platforms for care delivery, documentation, and coordination.
