Job summary

CBI Home Health is seeking an Occupational Therapist to serve clients in Kitchener-Waterloo, with a salary per visit model and a $5,000 sign-on bonus. The role supports home-based care across the community, emphasizing patient- and family-centered approaches and flexible scheduling.

Qualified candidates are licensed with the College of Occupational Therapists of Ontario and able to travel within the region.

Qualifications

• Must be a registered occupational therapist with the College of Occupational Therapists of Ontario.
• Mobility to travel within the community – valid driver’s license and insurance.
• Client-centered care approach with empathy and passion.
• Strong communication and critical thinking in client care decisions.

Responsibilities

• Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Collaborate with a multidisciplinary care team to deliver integrated services.
• Support restorative health approaches with health coaching and self-management support.
• Utilize digital platforms for care delivery, documentation, and intervention.

Salary Range: The salary range for this position is $57.44 – $64.95 per visit
