Job summary

CBI Home Health in Hawkesbury, ON hires an Occupational Therapist to support patients at home through the Integrated Care Program. You will perform functional assessments, suggest home modifications, and collaborate with care teams to promote independence.

We offer flexible scheduling across full-time, part-time, casual roles and a comprehensive benefits package while you help clients regain independence in their own homes.

Qualifications

• Must be a registered member in good standing with the College of Occupational Therapists of Ontario.
• Registration in good standing with COTO or equivalent.
• Ability to travel within the Hawkesbury area for home visits.

Responsibilities

• Assess and implement OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Collaborate with an interdisciplinary care team to coordinate services.
• Provide holistic, restorative health interventions and health coaching.
• Utilize digital health platforms for documentation and care delivery.
