CBI Home Health in Whitby, ON is seeking an Occupational Therapist to support the Integrated Care Program in the Durham region. This role focuses on functional assessments, home modifications, and collaboration with care teams to enable safe hospital-to-home transitions.

The position offers a salary per visit with a $5,000 sign-on bonus and flexible scheduling options across full-time, part-time, casual, and independent contractor arrangements. Travel within Durham is required.

Qualifications

• Licensed as an Occupational Therapist in Ontario.
• Willingness to travel within the Durham area with a valid driver’s license.
• Strong communication and collaboration skills.
